Jevady incorporates a holistic approach to personal health and wellness by integrating Family Medicine, Preventative Medicine, Sports Performance, Therapeutic Recovery Services (PT+) and Wellness services into one comprehensive healthcare membership program that enables active adults to stay active longer, extend their personal longevity and achieve their full potential.

Can I use my insurance to pay my monthly membership?
No, Jevady memberships are cash-pay only. We do not accept health insurance—we work directly for our patients, not insurance companies. By removing third party interference, we are able to provide high-quality care at a more affordable cost. But you can use your insurance for care provided by other providers, such as episodic physical therapy by Klesis Health, or Specialty Care (i.e. Cardiac, Orthopedic, Endocrinology, etc.).
Can I pay my membership monthly?
Yes, you can pay your membership on a monthly basis.
Can I cancel my membership at any time?
Yes, you can cancel your membership at any time. To avoid being charged for the next month, you must provide written notice before your next billing date.
If you cancel within the first six months of your membership (or within the first six months after a renewal), a $150 cancellation fee will apply. If you cancel after the first six months, the cancellation fee is prorated, starting at $150 and decreasing by $25 for each additional month of membership, reaching $0 after twelve months.
Please note that if we receive notice after your billing date, you will still be charged for the current month, and we will not issue a refund for that charge.
Can I keep my current out-of-state primary care provider?
Absolutely, we are happy to consult with your out-of-state PCP.
What is included in the Optimal Wellness Services in the Annual Personal Longevity Plan?
The Optimal Wellness sessions include personalized health coaching and functional nutrition designed to support longevity and overall wellness. Each year, every member receives three Optimal Wellness sessions, including one comprehensive 60-minute evaluation and two focused 30-minute follow-up sessions. Additional sessions may be scheduled at the patient’s convenience for an additional fee.
